Getting started is really quite simple.  Just download our Information Worksheet, Statement of Counseling Services AND Privacy Statement, fill them out and fax or mail them to us (Fax number is at the top of this page, address is at the bottom).  Once we have received all three you can call us to schedule a face-to-face or telephone interview to discuss your options.
